CDSM for Health Professionals
In recognition of the important role of Exercise Physiologists in providing services to Australians with lifestyle related diseases, Active Ageing Australia have received funding to develop and deliver chronic disease self-management (CDSM) training to 100 Accredited and practicing Exercise Physiologists who work across Australia, with trainings to be held in South Australia, Victoria and New South Wales. This CDSM Training is supported by funding from the Australian Better Health Initiative: A joint Australian, State and Territory government initiative.
The aims of the project are:
- to strengthen the capacity of the Accredited Exercise Physiologist workforce to implement the principles of chronic disease self-management and lifestyle and risk modification in their professional practice for community clients
- a demonstrated increased capacity of 100 Exercise Physiologists from the workforce to support self-management for members within the community who are living with one or more chronic disease
- improved care planning and use of decision support tools relevant to Exercise Physiologists and their clients
- provision of more collaborative, patient-centred care by Exercise Physiologists to people with chronic conditions; and
- improved awareness of and implementation of self-management skills and behaviours, quality of care, and ultimately, health outcomes for patients.
